Become a SNACK Participant
If you, your child, or a friend is 6 -18 years old and interested in learning more about healthy eating and physical activity opportunities, please join our program. Parent permission is required if the participant is under 18 years old.
How do I sign up for an appointment?
There are two ways to get started with SNACK:

Provider Referral. You can get referral to the SNACK Program by your primary health provider at Physician’s Medical Center, Virginia Garcia, or Sunrise Family Clinic. All you have to do is ask your doctor for a spot in our program!

Self Referral. If you are not a pediatric patient at these clinics, or if you prefer to make the appointment yourself, you can call the Call Center at Physicians' Medical Center to set up an initial visit at (503) 472-6161. You will need to be ready to provide basic information including name, birthday, phone number, and a few others.
What is the first appointment like?
You will start off with an introductory office visit where you will meet your SNACK Intern, discuss your needs and goals, take an initial survey, and learn more about the SNACK Program. You are welcome to bring your family to this visit and any other visit.

¿Cómo hago una cita?
Hay dos maneras para involucrarse con SNACK:

Una referencia de su proveedor médico. Puede obtener una referencia para el SNACK Program de parte de su proveedor médico que trabaje en el Physician's Medical Center o en Virginia Garcia. ¡Solo tiene que preguntarle a su doctor que le ayude obtener un lugar en nuestro programa!

Una referencia propia. Si no es es un paciente pediátrico en estas clínicas, o si prefiere hacer su propia cita, puede llamar al Centro de Llamadas del Physician’s Medical Center para iniciar su primera visita al (503) 472- 6161. Necesita estar listo para proporcionar información básica como nombre, fecha de nacimiento, número de teléfono, y algunos otros.
¿Cómo será la primera cita?

Become a SNACK Intern
We welcome your interest in working as a SNACK Intern! We currently host 8-10 Linfield College students as Interns each academic year. Our Interns get amazing opportunities to grow as mentors and young professionals. They teach all of our Family Nutrition Education Sessions, take measurements of children’s fitness, assist families in setting and tracking reasonable goals related to food and exercise, record clinical visits in electronic health records, provide activity sessions and cooking workshops for children, and act as a mentor and role model.
Our Interns receive high quality training in health education and public health, clinical experience, opportunities for advancement, and a letter of recommendation for career or graduate school upon completion. Perhaps the most rewarding part of being a SNACK intern is the feeling of being a part of the community and positively influencing a young person's life. How do I become a Volunteer?
Not a Linfield Student, but interested in getting involved? Consider becoming a SNACK Volunteer! Volunteers have helped with a number of projects, from providing exercise classes to producing community education events. If you have talents or interests you’d like to contribute to the SNACK Community, please contact the SNACK Program Coordinator.
How do I become a Community Partner?
If you are part of an organization who may be interested in supporting the SNACK Program, we welcome your groups to become a Community Partner. We are happy to come and give a presentation about our work, discuss collaboration or accept donations! Please contact the SNACK Program Coordinator for more information.
Partner Spotlights:
SNACK is housed at Physicians' Medical Center in McMinnville, where we work closely with pediatricians to provide a seamless transition from wellness checks to SNACK appointments. We work closely with McMinnville Parks & Recreation for events including Mac Monster Mania and Commit to be Fit Healthy Lifestyle Fairs, our SNACK Pass fitness program, and Kids On the Block after school program. We collaborate with classes and individual students from Linfield College every semester to deliver services and programs. We host monthly cooking workshops at the YCAP Food Bank Kitchen, the perfect space for kids and families to learn about nutrition and cooking together.
